1. Click on the link above. [NOTE: A dialog box will appear asking you whether to disable the macros contained in the spreadsheet. Click on "Disable Macros" or "No" and the spreadsheet should appear.]
   
   
2. Important! When the spreadsheet opens, click "File" and "Save As" to save the file to a folder on your hard drive.
   
   
3. "Open" the file from your hard drive in Excel.
   
   
4. Start with the "Instructions" tab at the bottom and read them carefully. You may want to print this page for convenience in completing the survey questionnaire.
   
   
5. Click on the "Company Information" tab and complete the information as requested.
   
   
6. Click on the "Pay Change Data" tab and complete the information as requested.
   
   
7. Click on the "Survey Questionnaire" tab and complete the information as requested.
   
   
8. Important! After you enter the information in the spreadsheet, "save" (overwrite) the file on your hard drive.
   
   
9. E-mail the file as an attachment to surveys@nae-online.org.
